DEVIL'S LAKE Field Trip

Final Payment is due by April 18, 2025. 

Please be sure to complete the following forms and turn them in to your students teacher as well:

  • Overnight Permission Form
  • Lodging agreement
  • Dietary Restriction Questionnaire
  • PCCS Student Medication Authorization Form (one form for EACH medication)

 

If making payment in installments, change the amount due to the amount you want to pay and place the item in your basket.  As a reminder, payment arrangements need to be APPROVED in ADVANCE by contacting iharrison@pccharterschool.org
